Travel Information

If you require any vaccinations relating to foreign travel you need to download and complete the travel questionnaire at the bottom of this page and return it to reception.  Telephone enquiries will no longer be accepted.

Please complete a travel questionnaire at least 12 weeks prior to your departure as you may require more than 1 dose of certain vaccines and your final dose must be given at least 2 weeks prior to travel to allow the vaccine/s to work.

Prescriptions for any vaccines/medication you require will be available to collect from the surgery 5 working days after you submit your completed questionnaire.  You should collect the vaccines from the pharmacy before booking an appointment with the Nurse to have your vaccinations administered as travel vaccines are not a stock item and the pharmacy may have to order it for you.

Some travel vaccines are only available on a private prescription and you will have to pay a charge at the pharmacy for these vaccines.  This is because not all travel vaccinations are included in the services provided by the NHS.
